Eligibility:

• Available to active duty, reserve, and retired military personnel, as well as veterans with an honorable discharge.
• Proof of service is required (see details below).

Customer Requirements

To qualify, the you must:

1. Submit proof of service to LTA. Accepted documents include:
   • Current Military ID, or
   • Separation paperwork (DD-214 of NGB 22) showing an honorable discharge.
2. Black out any personally identifiable information (PII) such as SSN, address, or service number before submission.
3. Enter documentation into this form, once verified, the customer will receive their $100 instant rebate and be eligible to submit their mail-in rebate.
